I am agree with Graham, the best format is certainly 
YYYY/MM/DD (or YYYY-MM-DD).

It help in case of sort.

At your question, I think than is better for
documentation than date have always the same format.
It can be usefull, to use this format into the code
files too.

One of the advantage is than it give an easy way to
give a version number to the file : the date of
submission.

A last remarks about history section (for this point
you have two different school):
* currently, the history order is older modifications
to  newer.
I think is better if the newer modifications are given
 in first position, because the reader can immediatly
determine, what is new and when.

The only problem is than the history part couldn't be
update only by append a new line at the end of the
list.

* It is difficult to determine the list of new
function since the last 'official' released of the
manual.
I think than we must add the "release" submission date
with the version number like :

...<here we have all history>
2002-11-16 Manual version 0.2.7c submission

or (and perhaps better :-?) add in the history only
the release submission date, following by the list of
modifications (dated or not) since the previous
released, like :

...
2002-11-16 Manual version 0.2.7c
    * Color modification (2002-08-27)
    * Add new shift and rot, and clean ISA options
(2002-09-08)
    * Add widen (2002-09-12)
    * Add madd, msub, mschg, cshift (2002-09-14)
    ...
