Log Message:
Well, that took longer than I thought:

- Fixed most of the FIXMEs.
- Protocol changed so that timing constraints work correctly.
  Now deadlines are always at the end of a period, which makes
  some things simpler.
- Account for replays.
- Added subsection on end-to-end receipts.
- Added subsection on trust requirements for witnesses.
- Fixed several errors in sections 7 and 8.
- Made section 8 model slightly more realistic (assume p_bad > 0,
  not p_bad = 1).

I'm pretty much done now, I think.
