wow a chapter in an O'Reilly book would be really cool.
looked pretty good on the whole, some minor comments:

this also makes it hard for good nodes to gain initial trust. This
causes the system to start very slow, and to gain new nodes more
slowly than is desired.
I keep feeling like what paypal does is either relevant to the whole
"what is micro payments, and how might they work" section, or that
we should mention why it's not relevant.
you have a section on "reputation and trust networks" with no
examples. both freehaven and ebay come to mind.
