Use the same number for every pad that needs to be considered the same with 
regard to the netlist.

How would pcb know, which not numbered pads it should consider as a group?

You can add the extra elements directly in pcb. They will be ignored by 
"update-rats" no matter which pin numbers their pads contain.

Nothing better, than the two workarounds you already considered. 

IIRC, the issue was discussed on the list while talking about the 
difficulties of arbitrary rotation angle. No patch seems to have sprung from 
this.
