Draw the outline of the hole with lines in a layer called lineout. Tell
the fab house about it in a README file.

They can easily mill arbitrary paths through the pcb, including the edges
of a rectangle hole. The corners of the rectangle will be a bit rounded,
of course. Most fabs charge extra for additional milling.
