LBF forms (take a peek)

Then click on 'register'.

Pages 1 and 2 are pretty standard as forms go, page 3's options are
generated dynamicly from the LBF DB.  I'm even learning SQL :-(

Feel free to tell me what you think of pages 1 & 2, those are done
basically done.  Page 3 is still devel.

Oh, and yes, I know the top bars are broken.  Jason is fixing it for me
(thank you Jason).
