OK.  Scratch that, the script wasn't broken... somehow I either missed
adding the script to run level 3 or it got removed.  

I created the sym link /etc/rc.d/init.d/qmail to /etc/rc.d/rc3.d/S80qmail

everything should be fine upon reboot now.
