 Somewhere in the whole channel_closed() call stack ... or possibly
 somewhere else called from connection_or_about_to_close ... we need to set
 the channel pointer NULL.

 I have a brute-force kludge at bug9602_kludge, which is done against
 0.2.4.  It should also merge cleanly into master.
