 For 0.3.0, I would suggest that we refactor everything that changes an
 AP_CONN's state, and everything that changes the pending_entry_connections
 list, to make sure they cannot be wrong.

 For 0.2.9, that seems kinda big.  Maybe we should just tune this warning
 down a little bit in 0.2.9?
