I noticed this earlier, but with this incident
it's appears nearly certain that a requirement
for the failure is that the consensus weight
increase sharply. Happened with these recent
two incidents (consensus weight is oscillating
between ~10000 and ~18000) and I noticed it
with at least one other. Might go back and
look at the consensus record for all five
failures.
Perhaps the bug is in some code that runs
only when the consensus goes up. That would
certainly make it difficult to reproduce.
