Comment:

 We fixed the cases we know about of this one; I've made bug #2179 to
 summarize the "lumpy traffic" cases I noted two comments above.

 I've also added #2180 to note that we should do more testing for all
 fairness types.

 If you find a new stream unfairness bug, please add or reopen a new child
 ticket, rather than adding more comments here; it's getting pretty
 confusing.
