Both. Look at my headers (Apple Mail):

Received: from [134.76.55.100] (helo=[10.100.145.215])

When using tor, 134.76.55.100 will be the tor exit node ip, and  
10.100.145.215 will still be your local client ip.

Yes, it doesn't make sense to use tor with a normal mail-client. But  
if you are behind a NAT router, it's not as bad as it looks first.
