Just register. They send pass & login by email, then you can use online form
to request board. All you need - zip-file with gerbers, generated by PCB
(*.gbr, *.cnc).
PS: I don't understand that form too :).
PPS: Just another firm: pcbtech.ru. Bestest, compare to tepro.
