The surface mount devices are usually (probably always) in mm or mm/100,
through-hole devices can be either mils or mm. DIPs will be in mils but most
other devices should be in mm or mm/100.
Before using a footprint you should verify that the footprint matches the
manufacturer's package specification for your component.
