You're still having the problem that the clearance for the *line* is
corrupting the *via*.  That's what happens when you try to stay 10 mil
away from a 15 mil line, while at the same time trying to connect to a
15 mil hole with a 10 mil annulus.

Put the crosshair over the line and press 'j' to join it with the
polygon, just like you did for the other two.
