Footprints in pcb are always embedded without any reference where they
came from, except for their name.  If you change the footprint file, you
need to delete the old instance and replace it with the new one.  DJ
described how to do that with one click yesterday.

Since you renamed the footprint, both your changed copy and in the
schematic, running gsch2pcb will delete the old instances and put the new
ones into the .new.pcb file.

File->Import Schematics should do something similar, but did not use
that yet.  That's for my next project to try.

Else, as long as refdes and footprint name of an element agree with what
the schematic says, an element in the pcb file will not be changed, even
if is something completely different than what a new instantiation
would provide.
