The KDE bug tracker (http://bugs.kde.org) is rather nice, especially the way 
that CVS log messages relating to a bug are automatically added as comments 
to that bug so you can see developer activity on them. :)

Not sure which bug tracker software they use, might be a custom-rolled job.
