I made one of these for a recent project by using a 25-pin and just adding
the additional pins and moving the mounting holes.
That is easily done with a text editor. I've attached mine and made
quite a few more. Once you have done, one or two, it gets easier.

Read the docs and take a single opto and make your own symbol. It's fairly
easy. Place an opto from the library. I know there is one there as I've used
it. place on your blank schematic. Use hierarchy and push down into symbol,
select everything and copy to a buffer. Create a new schematic, paste two
copies of the buffer and do all the pin and attribute edits and save it as
"dual_opto.sym" in your own library area.
There is a doc on this.
