I'd prefer a comprehensive answer to: 
"What is the recommended way to do boards with multiple copper layers?"

Seems like a historic echo to me.

How do I define the physical outline of my layout?

By the way: If gsch2pcb were to include the outline layer in the 
initial layer stack, this source of newbie confusion would be defused.
