At work (well before my time) they once had a really hard to diagnose
motherboard problem that turned out to be caused by a BGA footprint
where one of the "dogbones" wasn't actually connected -- there was a
pad, and a via, but they weren't connected.

Sounds like SOMEONE tried to do it by hand!
