I think a combination of the two for that paticular link.  Have the
index at the top that links to anchors in the page.  If possible.  If
not, and what's up there is all that's available, then I say the
all-in-one.

However, for longer document, even if the mixture is available, I'd say
the index-only is the way to go.  I'd probably make the cut-off
somewhere around 500K or so...

Thats my opinion anyway...
