I've been collecting some data about fabs, here's some hints (I've not
done business with any of these but 4pcb, this is just from reading
their web pages).  The numbers are line/space/mindrill, cost does NOT
include shipping, but I tried to estimate a 12sqin board (my
spreadsheet has 10 and 20sqin columns).

no mask or silk:

Futurlec - 21-28 for one, 10/10/24
pcbpool - 22-27 for one, 6/6/12 - charge extra for gerber data.
custompcb - 38 for two, 8/12/24
4pcb - 50 for one, 6/6/15 (my furnace board used this service)
apcircuits - 60-71 for two, 7/7/17

with mask and silk:

pcbnet - 25 for one, 7/7/15 (new customers only)
olimex - 44 for one, 8/8/28 (faster delivery if 10/10)
batchpcb - 40 for one, 8/8/20
pcbpool - 62 for one, 6/6/12 - charge extra for gerber data.
futurlec - 57-64 for one, 10/10/24
myropcb	- 80 for 4, 5/5/12 (up to 60in2 at this price)
4pcb - 99 for 3, 6/6/15 (initial protos for the furnace board used this service)

4pcb at least will let you go from proto to production without
additional tooling costs, others probably will also.
