Put in a via without a thermal.  The only reason you'd need a thermal
is if you're hand soldering it, and even then you'd just need a bigger
iron tip.  Wave soldering might have a problem with it too, but reflow
soldering (the only way to solder that pad *anyway*) won't because
that heats up the inner layers also.

Yes.  Do that :-) In fact, do it a couple of times, to make a better
heatsink.
