Setting the size larger than needed provides more room to lay out
components.  You may want to start with an area twice as large as needed
and then move everything to the top, left-hand corner at the end ... when
you resize the board to the final dimensions.

Drawing silk on the "outline" layer  (if you don't have it, create it)
will output a GBR file with your board outline.  This needn't be square. 
It can be whatever shape is needed.  It's a handy way go.  Your board shop
will know what to do with it in most cases.
