Have done both - no change.

I have also noticed, that the first time <i have installed the browser, there was an entry in the start menu. After I have deleted the Tor Browser folder and Start Tor Browser Icon it never appeared again (in the Start menu). Do I have to fix also something in the registry? Why doesn't it appeart anymore?

Does that also happen after you click 
the arrow next to the onion and click on "New Identity"?

if that does not solve the problem:

Can you try to download a fresh torbrowser into a new folder 
and see if that solves your problem?
