have you tried driving it with a lower amplitude (low enough to not
saturate the outputs) square wave and verifying that you reach the 20
V/us slew rate then?
If it were me, I'd step back and make sure that nothing is goofed up
with the measurement (try measuring a square wave from a pulse generator
and making sure you don't have any speed issues there). Then I'd make
sure that I could see a 20V/us slew rate with a square wave signal that
is small enough to not saturate the amplifier.
