Ok, here's a random thought.  What if the hotplate reflow delaminated
one of the layers?  You'd still need a cause, but it would make the
cause's minimum size smaller.

Of course, there's no *visible* effect on the pcb from the hotplate,
not even a mild yellowing on the back.
